We are so excited you want to join us on our journey to leave a legacy of smiles!

Before you begin, please note that there is a $100 application fee for processing and background checks. Everyone will need to attach copies of their photo id and resume or cv. If you are a dentist or medical doctor, you will also need to attach copies of your License and your Diploma. Please have those ready before you begin so you do not lose your progress.

We also highly suggest that you have looked through the volunteer guidebook pages before you apply.

Each person attending must fill out a their own application.

Dental Legacy Foundation will reach out to you via email with their decision on your application.
